Patient Registration

Before you come to MedCenter Ambulatory Surgery your surgeon will review the details of your surgery and will give you detailed pre-operative instructions. Representatives from the business office will contact you prior to the surgery to confirm necessary personal and financial information. You will also need to speak to the pre-op nurse prior to your surgery. If you have not been contacted at least one day before surgery, please call the Center.

The following list of forms are available for you to print, read and fill out at your convenience. You will need Adobe Acrobat Reader to view and print these forms.

• Patient's Bill of Rights and Responsibilities
• Patient Notice of Advance Directives and Physician Ownership
• Pre-Op Assessment
• Consent for Admission and Treatment
• Notification of Herbal Use and Surgery
• General Post Operative Home Care Instructions*

* Depending on your specific procedure(s), your doctor may give you different or additional post-operative instructions. Please follow your doctors instructions.

If you have any unexpected complications, such as excessive bleeding, medication reactions, high fever, etc., please contact your doctor immediately or proceed to the nearest emergency care facility.
